Employment Benefits
Cardiodiagnostics is pleased to offer a comprehensive benefit plan for all employees. Full-time employees are eligible for all benefit
programs though some benefits require a minimum of ninety days employment to meet eligibility requirements. Part-time employees
are subject to a limited benefit plan. Please refer to the benefit lists to the right on this page for each type of employment.

BEREAVEMENT LEAVE: Cardiodiagnostics will pay up to 3 days for Bereavement
Leave for the death of an immediate family member.
UNEMPLOYMENT INSURANCE: Cardiodiagnostics carries unemployment insurance.
Employees who lose their jobs may be eligible for unemployment insurance
pursuant to applicable employment security laws.
JURY DUTY LEAVE: In consideration of Jury Duty service as a civic duty,
Cardiodiagnostics pays employees called for such duty up to 10 days of their
regular pay less the amount they are paid by the court while they are on Jury
Duty.
WORKER'S COMPENSATION INSURANCE: Cardiodiagnostics carries Workers Compensation
Insurance. This offers protection for employees in the event of any job-related
injuries or illness.
SOCIAL SECURITY: Cardiodiagnostics matches all employee contributions to the
Federal Social Security System. You will be covered by Social Security the day
you begin working. We will withhold the employee's portion from your
compensation as required by the Federal Insurance Contribution Act (FICA). As
your employer, Cardiodiagnostics will contribute to your Social Security account
an amount equal to the amount withheld.

CONTINUING EDUCATION EXPENSE (CME) & PROFESSIONAL FEES/DUES: Cardiodiagnostics
will pay expenses for these items for full-time employees and one third of the
benefit amount for part-time employees as follows: (1) up to $1500 every 3 years
for Ultrasound, Nursing, or Nuclear Technologist staff and, (2) up to $500 every 3
years for Medical Assistants and Administrative staff.
PROFIT SHARING PLAN: At its' sole discretion, Cardiodiagnostics may choose to
fund the company's Profit Sharing Plan on an annual basis. Eligibility requires
completion of one full year of employment and the employee must work a minimum
of 1000 hours in the calendar year for which the funding is made. This plan is
also subject to a graduated, 20% per year, vesting schedule.
401(k) PLAN: Cardiodiagnostics offers a 401(k) savings plan for its' employees.
Pre-tax income may be voluntarily deducted at the employee's discretion for
tax-deferred investment in a wide variety of investment vehicles of the
employee's choosing. The plan is subject to IRS maximum contribution limits.
FLEXIBLE SPENDING ACCOUNT (FSA) PLAN: This voluntary plan is available through
payroll deduction. The plan allows employees to pay for medical and dependant
children daycare with pre-tax income resulting in a potentially significant
federal income tax savings.
SUPPLEMENTAL INSURANCE: A variety of voluntary supplemental insurance plans
(such as accident and cancer plans) are available for purchase by full-time
employees through payroll deduction. Spouse / family coverage is also available
with these plans.
DISABILITY INSURANCE: Short-term and long-term disability insurance is provided
for full-time employees. No coverage is available for family members.

HOLIDAY PAY AND PERSONAL DAY OFF: Full-time employees are eligible to be paid
for a day off on Cardiodiagnostics' 8 recognized holidays (Memorial Day,
Independence Day, Labor Day, Thanksgiving Day, the day after Thanksgiving Day,
Christmas Eve Day, Christmas Day, and New Year's Day) provided they have
averaged 32 hours per week for the 6 months prior to the holiday date. In
addition, employees may take a paid Personal Day for any purpose of their
choosing each year.
PAID TIME OFF (PTO): Paid time off is accrued by all employees, full-time and
part-time, based upon the regular (non-overtime) hours they work. The accrual
rate is based upon the years of service to Cardiodiagnostics by the employee,
beginning at a rate of 15 days per year for a 40 hour per week employee and
increasing gradually to a maximum rate of 25 days per year for a 40 hour per
week employee after 15 years of service.
DENTAL INSURANCE: All full-time employees are eligible for this voluntary
benefit. If elected, coverage begins on the first day of the month following
completion of 90 days of employment. Premiums are paid through payroll deduction
and family coverage is available.
VISION INSURANCE: All full-time employees are eligible for this voluntary
benefit. If elected, coverage begins on the first day of the month following
completion of 90 days of employment. Premiums are paid through payroll deduction
and family coverage is available.
LIFE INSURANCE: A $15,000 life insurance policy is provided for each
full-time employee enrolled in Cardiodiagnostics' health insurance plan. No
family life insurance coverage is available.
